Children’s Blended Case Management

Blended Case Management services strive to help children and adolescents live successfully within their family unit and the community.  Services are provided based on consumer need. BCM is required to meet with the family face-to-face, a minimum of once per month.  Additionally, crisis services are available 24 hours a day, 7 days  a week.

A child’s assigned case manager will travel to the child’s home, school, medical appointments or other sites where the individual needs support. The case manager coordinates all services needed by child and family.

BCM services are available for children:
•Diagnosed with a mental illness or emotional disturbance.
•3 to 21 years of age. Young adults ages18-21 are only eligible for BCM if currently enrolled in a secondary level education system.
•Enrolled in Medical Assistance.
AND must meet one of the established treatment criteria listed below:
•Children having six or more days of psychiatric inpatient treatment within the past twelve months.
•Children recommended for services by local providers, that without BCM, would result in an out-of-home placement (Community Inpatient, Foster Home, Juvenile Court placements, or children returning from out-of-home placement).
•Children who with their families, are receiving or in need of services from two or more human service or public systems (Education, Child Welfare or Juvenile Justice).
•Children who are recommended as needing mental health services by a local interagency team

BCM services include:
• Development and implementation of strengths-based assessment
• Development and maintenance of community linkages
• Participation and coordination of treatment team services
• Crisis intervention

BCM services are based on the needs of the child and family. The focus of BCM is on meeting basic human needs in the following domains:
• Physical
• Emotional
• Spiritual
• Vocational
• Social
