Blended Case Management (BCM) provides services to individuals with behavioral, psychological or developmental challenges. BCM care is customized to meet the needs of each individual.

Blended Case Managers travel to the consumer’s home, medical appointments or other sites where the individual needs support. The case manager coordinates all services needed by consumers, both within Salisbury Behavioral Health and by other providers.

BCM helps provide, or link consumers with the following:

  • Housing/living
  • Education/vocation
  • Income/benefits
  • Money management
  • Mental health treatment
  • Drug and alcohol treatment
  • Medical treatment
  • Socialization/support
  • Local transportation services
  • Assistive technologies
  • Nutritional education
  •  Accessibility in accordance with the American with Disabilities Act
  • Additional services as needed

The depth and frequency of case management services is based on the individual’s assessed level of need. Case managers are available 24-hours a day, seven days a week to deal with emergencies and crisis intervention.


Blended Case Management is offered in: Berks, Carbon, Lehigh, Monroe, Northampton, and Pike Counties
