Supported Living

The supported living program (SLP) seeks to provide a structured recovery environment for adults dealing with the challenges of persistent mental illness. Structure is offered through Crisis Support, Medication Management, Training and Support, and Peer Support as identified by the resident’s individual recovery service plan.

The Supported Living Program (SLP) empowers people to make life choices, attain life goals, build relationships, and participate in the community life to the best of their abilities and in the most integrated and independent manner for the individual and her community.

The Supported Living Program (SLP) provides mental health services that are resident-centered, culturally competent, individualized, flexible, coordinated, accountable, and strength based with the goal of providing the opportunity for people to recover with the recognition that the recovery process is unique for each individual.

The Supported Living Program (SLP) is designed and operated to specifically support individuals dealing with the challenges and difficulties of persistent mental illness. The Supported Living Program  supports individuals to live as independently as possible through the provision of training and assistance in the skills of the community living and by serving as an integrating focus for the individual’s rehabilitation.

Individuals in the Supported Living Program are adults 18 years and older. Admission to the Supported Living Program  is based on a referral process as established by the County Office and Salisbury Behavioral Health.

The Supported Independent Living Program (SIL) in Montgomery County is an apartment setting with one other roommate with staff available 8 hours per day.

Service Description:

The population is adults 18 years and older.  Admission to the Supported Living Program is based on a referral process as established by the County office and Salisbury Behavioral Health, Inc.

The Supported Living Program is designed and operated to specifically support individuals dealing with the challenges and difficulties of persistent mental illness.  The Supported Living Program supports individuals to live as independently as possible through the provision of training and assistance in the skills of the community living, and by serving as an integrating focus for the individual’s rehabilitation.

Staffing at Supported Living Program:
• A Program Director for 40 hours per week
• A Clinical Care Manager to support development of care plan
• A Care Manager to support medical appointments and services
• One staff per shift to help with daily routine, crisis situations, medication management, etc.

 

Supported Living Program is offered in: Montgomery County, Lehigh County and Northampton County
